If you have a concern about your child’s motor, sensory, communication or feeding skills, your first step is to discuss your observations with your pediatrician.

If a therapy consult is recommended, the next step is for you to contact our Family Service Coordinator, Barb Morin at 847-510-5633 or email us at familyservices@pathwayscenter.org.

Our Family Services Coordinator will ask you a series of questions including:

• What is your child’s level of function?

• Do you have any primary areas of concern?

• Is there any significant medical history we should know about?

• Does your child use mobility aids or special equipment?

• Is your child currently receiving services? And if yes – What are they?

 

If it appears that your child’s needs can be addressed by Pathways therapy staff, you will be asked to provide your availability for scheduling an evaluation and potential treatment times as well as insurance information.

Your child’s specific needs will be considered when selecting a therapist for your child. Whenever possible, the therapist conducting the initial evaluation will also provide treatment services if recommended.

Once a date for the evaluation is confirmed, an Intake Application and Insurance Registration form will be forwarded for you to complete and bring to Pathways on the day of the scheduled evaluation. We will perform a benefit check and share that information with you in advance of your evaluation.

Our Family Services Coordinator will greet you and your child on the day of the evaluation and will introduce you to the evaluating therapist and business account manager. If therapy services are recommended, she will assist in contacting your pediatrician to obtain a prescription for treatment services.

Screenings

If you or your pediatrician are uncertain whether therapy services are indicated for your child, Pathways offers a 15-minute screening at no charge. A therapist will observe your child and ask you a few questions to determine if a comprehensive evaluation is recommended. To take advantage of this free service, contact Barb Morin.
